Control of blood glucose in type 2 diabetes without weight loss by modification of diet composition
Background:
Over the past several years our research group has taken a systematic, comprehensive approach to determining the effects on body function (hormonal and non-hormonal) of varying the amounts and types of proteins, carbohydrates and fats in the diet. We have been particularly interested in the dietary management of type 2 diabetes. Our objective has been to develop a diet for people with type 2 diabetes that does not require weight loss, oral agents, or insulin, but that still controls the blood glucose concentration. Our overall goal is to enable the person with type 2 diabetes to control their blood glucose by adjustment in the composition rather than the amount of food in their diet.
Methods:
This paper is a brief summary and review of our recent diet-related research, and the rationale used in the development of diets that potentially are useful in the treatment of diabetes.
Results:
We determined that, of the carbohydrates present in the diet, absorbed glucose is largely responsible for the food-induced increase in blood glucose concentration. We also determined that dietary protein increases insulin secretion and lowers blood glucose. Fat does not significantly affect blood glucose, but can affect insulin secretion and modify the absorption of carbohydrates. Based on these data, we tested the efficacy of diets with various protein:carbohydrate:fat ratios for 5 weeks on blood glucose control in people with untreated type 2 diabetes. The results were compared to those obtained in the same subjects after 5 weeks on a control diet with a protein:carbohydrate:fat ratio of 15:55:30. A 30:40:30 ratio diet resulted in a moderate but significant decrease in 24-hour integrated glucose area and % total glycohemoglobin (%tGHb). A 30:20:50 ratio diet resulted in a 38% decrease in 24-hour glucose area, a reduction in fasting glucose to near normal and a decrease in %tGHb from 9.8% to 7.6%. The response to a 30:30:40 ratio diet was similar.
Conclusion:
Altering the diet composition could be a patient-empowering method of improving the hyperglycemia of type 2 diabetes without weight loss or pharmacologic intervention. (more…)

Absorption and metabolism of conjugated ?-linolenic acid given as free fatty acids or triacylglycerols in rats
Background:
Conjugated linoleic acid (CLA) is a group of polyunsaturated fatty acids which have been extensively studied in the past two decades. However, conjugated octadecatrienoic acid such as cis-9,trans-11,cis-15 and cis-9,trans-13,cis-15, recently identified, have not been extensively investigated. This work presents bioavailability and tissue incorporation of a mixture of conjugated octadecatrienoic (CLnA) acids ingested as free fatty acids (FFA) and triacylglycerols (TAG).
Results:
Male Wistar rats were fed rumenic acid (RA: cis-9,trans-11 18:2) and a CLnA mixture (cis-9,trans-11,cis-15 18:3 and cis-9,trans-13,cis-15 18:3) as FFA and TAG for 8 days. RA and CLnA were both totally absorbed when given as FFA as well as TAG. Both isomers of CLnA as FFA or TAG were incorporated into neutral lipids. Metabolites up to 22:6 conjugated isomers were present in liver and plasma phospholipids of rats fed the CLnA diets.
Conclusion:
Finally, CLnA are as well absorbed as RA in vivo and their incorporation into tissues and bioconversion are similar when ingested as FFA or as TAG. (more…)

There is a common misconception that you have to starve yourself to lose weight. This is simply not true! On the contrary, you must eat to lose weight. Glucose is the only fuel your brain uses and it is the brain that uses more energy than any other part of your body. If your blood sugar goes down, it starves the brain and a wide variety of symptoms can occur such as feeling weak and sluggish, drowsy, dizzy, confused, headache, irritability, trembling, rapid heart beat, and even anxiety and panic attacks. Carbohydrates alone do not make a healthy body. We need a balance of protein, carbohydrates, fat, water and exercise to maintain health. The biggest difficulty that we have with weight problems is over indulging in high glycemic foods. These are the foods that contain too much sugar or foods that are refined carbohydrates. If you over indulge in carbohydrates, (which produce insulin) then the excess glucose gets stored as fat. You can counter that by eating protein with it, (which produces glucagons). Glucagons promote the utilization of fat to be used for energy. Insulin and glucagons in balance maintain normal blood sugar levels. A body in a high insulin state promotes excess stored fat and disease. The foods that are high on the glycemic index are foods such as the root family, potatoes, carrots, parsnips, rutabaga, yams etc. If you want to eat potatoes then I?d opt for the yams or sweet potato (without the honey and brown sugar). They are the lowest on the glycemic scale. Also rice, pasta, corn, most breads are high on the scale as well as some fruits like bananas, apricots and pineapple. Some low index foods are vegetables such as broccoli, the lettuce and cabbage family, various beans, dairy and soy. I would also like to make a mention about fat. Fat in it self is not the bad guy. Our diets require fat to remain healthy. Fat provides fuel for the body plus it helps in the development of cell membranes and hormones and also helps to insulate our bodies. Fat in our foods is what gives us that satisfied feeling. You don?t need very much fat intake. Stick with good fats like olive oil and stay away from saturated or trans fats. Portion size is very important. The rule to remember is; if you have approximately 3 oz. of protein and you have a high index carb. then eat only 3oz of that carb. It is a one to one ratio. A protein and a low index carb. would be 3 oz of protein and 6 oz of carb. It is a 1 to 2 ratio. If you eat 6 small meals per day in the correct ratio then you should easily be able to lose weight.
